Hi, I am very much in need of help to complete an important form from state board to show proof of employment for the year 2020 2021 when I worked at HCR Manor Care. The form needs to be completed and sent directly to state in order to show proof of employment in order to be considered for loan reduction. Please I need a valid email to send the form as soon as possible to get it filled out and sent directly to the email at the bottom of the form. Thank You.

I have not received anything from you and it has been months. Please contact me asap to resolve this matter or I will have to let my lawyer handle this. Thank you Janette Farmer. 561-541-1678

I arrived at Heartland/Canton 7/9 and eventually transferred to Glacier Hills Rehab center
July 10, 2021
6am call light on
7:50am aide walks in to ask what I needed-need bed pan-aide says ok and they will be right back-7:15am aide arrives- I already wet the bad-embarrassing and humiliated-I am a continent adult-this happened on at least 3 occasions
I used a regular bed pan for days-everyone I asked said that they didn’t have any-What ? No fracture pans in rehab? Absurd.
Finally Donna found one in the equipment area???
I asked for distilled water for my CPAP machine-two nights in a row I hear running water-the next night the aide did get the distilled water downstairs-weird no one had gone down before-
Needed my bed changed-call light on for two hours-the aide told the nurse I refused to let him make bed. What??
The nursing care, overall was excellent.
I could hear the aides argue in the hallway on several occasions-
I was told that passing trays was a priority over answering call lights so my sweet roommate and I both wet the beds that day
I felt that the aides did not want to be there- when answering call lights, there was the rolling of the eyes or grunted “oks”. Seemingly unhappy staff.
Suggest:
Education
-on total hip precautions
-fracture pans
-that a system be in place so it doesn’t take a third aide answer to get help
-appropriate language for the hospital setting
-on the real needs of the patients
not just eye rolling when answering light
-Care workers are hear to help patients and not hear (in the hallway) arguing about their jobs, patients, and unrelated issues.
-there were a few aides who were awesome, (I wish I had their names)
-not having enough info about the staffing guidelines-need to have an aide not involved in passing trays, answer call lights as their only responsibility
-ask the workers what would work and have an in service on customer service. The job IS to care for patients. They need YOUR care.
God bless all
Sincerely
Sylvia Ertman

I was a temporary resident at your Sewell facility for approx. 6 weeks. Receiving IV’s three times a day on 8 hr. intervals and getting PT & OT once a day, 7days a week. My experience was enlightening and rewarding. Which I retained and use as a format in my daily recovery at home and hopefully for the rest of my days. My compliments to your RN/LPN’s for their professionalism and candor. I have special appreciation for the duties of your CNA’s and the many hours they put in. The COTE’s, PT’s & OT’s have my special thanks for teaching me to walk with partial bearing, exercising moderately not to overwork sore joints using the right equipment. I would highly recommend Manor Care in Sewell to anybody, as it was to me by my In-laws.
