There is a resident at the BH in Pagosa Springs CO named David Swindells. BH has informed me they can no longer care for this resident. I received an eviction notice for Mr. Swindells from BH Pagosa Springs on 11/15/22. The notice requested Mr. Swindells needs to be transferred from BH on or before 12/01/22.
The termination notice states “we provide our residents a 30 day notice for termination of services”. I received the termination notice on 11/15/22 and BH Pagosa is requesting Mr. Swindells be relocated by 12/01/22. That is 15 days.
I have requested BH Pagosa send referrals to 3 other locations ASAP. Two have received the referral one has not. The facility that has not received the referral is the location we would like to move Mr. Swindells too, if he is accepted.
Please contact BH Pagosa and light a fire under some ones butt. 12/01/22 is days away. I am trying to comply with the 12/01/22 deadline.
